Rhinoplasty without implants, also called implant-free rhinoplasty, reshapes the tip or bridge with your own tissue, such as ear or septal cartilage, or with no graft for a mildly low tip. Who is a good candidate for implant-free rhinoplasty?
- You would rather not have an implant, or worry about inflammation or an implant showing through the skin
- Your bridge is fine but the tip is low or lacks shape, which the clinic lists as a suitable case
- Your nose is short or upturned and the tip needs firm support from your own cartilage
- You want a modest change to the bridge line rather than a lot of extra height
When it may not be the right choice
- You want a much higher bridge. Ear and septal cartilage come in small pieces, so rib cartilage rhinoplasty or an implant gives more height
- You have had nose surgery before, especially if cartilage from your septum was already used. Revision rhinoplasty is planned differently
Which method fits you?
The clinic chooses the material for each part of the nose by what it has to do. A mildly low tip may need no graft. Where height or support is needed, it uses ear cartilage, septal cartilage or your own dermis: soft ear cartilage where the tip has to move, straighter septal cartilage where a firm line is needed.

What happens on the day of implant-free rhinoplasty?
- Consultation. The surgeon checks how far the tip needs to move and how much cartilage your septum can spare. The clinic’s 3D CT scan of the nose helps show the septum.
- Anesthesia. For its implant-free tip and bridge work the clinic lists sedation and about 2 hours, with no overnight stay. Plans built from rib cartilage are a different operation, done under sedation or general anesthesia.
- If a graft is planned, cartilage is taken from the ear or the septum, then shaped and stitched into the tip or along the bridge through incisions inside the nostrils or across the columella, the strip of skin between them.
- The donor site is closed, with a dressing on the ear if cartilage came from there. A splint protects the nose, and you go back to your hotel the same day.
What is recovery like after implant-free rhinoplasty?
- Days 1 to 3
- The tip is tight and swollen and the nose is blocked. The ear feels sore if cartilage was taken there.
- Days 5 to 10
- Stitches come out in this window. Ask when your splint will come off.
- Weeks 2 to 3
- Bruising is mostly gone. The tip stays full for longer, especially around the grafts.
- Months 1 to 6
- Swelling goes down slowly around the grafts, and the tip can feel firmer than before surgery.
- Months 6 to 12
- The tip settles into its final shape. Tell the surgeon if a graft seems to have shifted or bent.
How long should you stay in Seoul after implant-free rhinoplasty?
Plan 7 to 10 days in Seoul. Stitches come out between day 5 and day 10 by the clinic’s figures, with 2 to 3 follow-up visits in that time, so plan your flight for between day 8 and day 10, after the last check. An ear donor site is a second wound to keep clean. As after any operation, if you become breathless or have chest pain, get emergency care first, then tell the clinic. Do the same if bleeding from the nose or the ear is heavy and does not stop.
What are the risks of implant-free rhinoplasty?
- Cartilage that bends (warps), shifts or loses some volume over time, changing the shape
- Less height than an implant or rib cartilage could give
- Soreness, a scar or a change in the shape of the ear where cartilage is taken
- Where septal cartilage is taken, bleeding or a blood collection inside the nose, a hole in the septum, or a septum left with too little support for the bridge
- A firm tip, and swelling that takes months to settle
- Edges of a graft that can be felt or seen under thin skin
- Infection, nosebleeds and a blocked nose
- A tip that settles higher, lower or firmer than planned, which can mean a second operation
What affects the cost of implant-free rhinoplasty in Korea?

Is rhinoplasty without implants safer than using an implant?
Not clearly. In a meta-analysis of nose bridge reconstruction, complication rates were close: 6.9% of people with their own grafts and 7.8% with implants had a complication, a small gap that does not settle which is safer. Complications still happen with your own tissue, including infection, warping, thick scars and a second operation, so the choice depends on your nose.
How much height can my own cartilage add to the nose?
Less than an implant or rib cartilage. Cartilage from the ear or septum comes in small pieces, which suit the tip and modest bridge changes. If you want a much higher bridge, the surgeon may suggest rib cartilage or an implant instead.
Will my ear look different after cartilage is taken?
Some people notice a change. In a study that asked patients about their ear after cartilage was taken, most were satisfied with the scar and reported no loss of confidence, though some noticed differences in its shape or symmetry. Ask where the incision will be.
What is tip plasty?
Tip plasty is a term some Korean clinics use in English for surgery on the nose tip alone, from tying the tip cartilages to adding your own cartilage. It does not change the bridge.
